Specific Conditions:
Adults (18+yrs): Bring along your ID/Passport).
Children (10–18yrs): Child’s ID/birth certificate + Parent / Guardian ID. If surnames are different, bring an affidavit from the Police by their parent giving you consent to bring the child in for the procedure, also bring a copy of their ID.
Children (6–9yrs): Same conditions as 10-18yrs
